It is located approximately 7 miles west of O'Neill, the county seat of Holt County.
The community was named for C.O. Sanders, who was one of the original settlers in the area. In the late 1800s, Sanders opened a store in the community and the community grew around it. In 1910, the small community officially became known as "Smithville-Sanders".
