Travel itinerary, if you have already made travel arrangements. Dates of your last five visits or trips to the United States, if you have previously travelled to the United States. You may also be asked for your international travel history for the past five years.

From an American English common usage point of view, that question is intended to mean list the countries visited in the last five years.
While granted for people who are used to other variations of English this might not be clear, the US Government tends to write out questions and information as if they were speaking with the average American not someone for which English is a second or third language.
