US: To replace a visa from outside the USA, you must apply in person at a U.S. Embassy or Consulate abroad. When applying for the replacement of a visa, you will need to provide a written account documenting the loss of your passport and visa. UK: You need to apply for a replacement using transfer of conditions. A police report or other evidence that satisfactorily establishes that the passport is no longer in your possession is required. https://www.gov.uk/transfer-visa
